Make an Appointment

To make an appointment please call our practice or fill out the appointment form below. Once submitted, you’ll receive a confirmation email, and we’ll get back to you on the next business day to schedule an appointment date and time.

Appointment Checklist

  • Your Doctor/Dentist referral to one of our specialists
  • A list of any medications that you are taking
  • Your Medicare Card/ Private Health Fund card
  • Any relevant diagnostic imaging such as CT Scans, MRI scans or other xrays
  • Any relevant reports from investigations such as Pathology and Audiograms (Hearing tests)
  • Please note: we are not a bulk billing practice
  • Please submit your new Patient Registration Form. The form can also be completed on the day of your appointment
